    About WyndRidge Health & Rehabilitation Center

    We realize there are many nursing homes to choose from, but we believe we go above and beyond in resident & patient care. WyndRidge has 157 dually-certified beds making our facility larger than the Tennessee state average of 116 beds. Our residents are cared for by a dedicated professional staff that includes the resident's Physician, Physicians Assistant or Nurse Practitioner, Registered Nurses, Licensed Practical Nurses, Case Management, Activities Staff, Registered Dietician, Respiratory Therapist, Pulmonologist, Certified Nurse’s Aides, Rehabilitation Therapist {that include Physical, Occupational, and Speech}, Wound Care Specialist as well as caring dietary, maintenance, and housekeeping departments. The Comprehensive Care Team develops a highly specialized care plan that is designed to meet each resident’s individual needs. Comprehensive onsite services are provided to residents, including full pharmacy services, intravenous therapy, nutritional support, pain management, and respiratory treatments. WyndRidge has one of the few, and the most up to date, Ventilator/Tracheostomy Units in the state of Tennessee, and is staffed appropriately for short-term and/or long-term Ventilator/Tracheostomy care.

We are a non-profit organization and volunteers are a vital part of our facility. We need volunteers in many different areas of the nursing home such as Activities, Fundraising, spending quality time with residents, including the 'No One Dies Alone Program', or our 'Memory Team Program', even administration and maintenance can use the help of volunteers or in kind donations of supplies/materials.

  • Information

    Checking In

    Prospective residents must supply the facility with signed physicians orders that state “Admit to our facility” and include all medications, labs, therapies, and treatments that may be required. The physician must also supply us with a History and Physical from within the past year as well as a chest x-ray from the past thirty days. Once this information is obtained we will have an interdisciplinary “Admission Meeting” to make sure that this facility can meet your loved one’s needs.

    We will then notify the family of when to bring their loved one into the facility. On arrival we will take the new resident to their room, and the nursing staff will perform a nursing assessment. The Admissions Coordinator will then take the responsible party to complete admission paperwork.

    On the following Monday:
    After admission you will be assigned a time for a “Care Plan Meeting”. It’s at that time you will meet the interdisciplinary staff that is providing care to your loved ones.

  • Ventilator and Tracheostomy Unit at WyndRidge

    Unique Respiratory Care Unit Celebrates Successful First Year of Operation

    WyndRidge Health and Rehabilitation Center in Crossville, Tennessee, has always tried to adapt to the changing needs of the community. November 2012, the first ventilator/tracheostomy resident was admitted to the 26 bed unit, allowing the facility the ability to take care of more residents on life support than most ICU's in Central Tennessee. Instead of local referrals being sent to other facilities in further locales, those persons can stay closer to home.



    A bulk liquid oxygen system pipes high flow oxygen along with a new compressor system providing hospital grade suction to bedside outlets. A state of the art Redundant Alarm System has been installed providing an extra measure of safety and sense of security for residents and their families. Care staff include licensed respiratory therapists, nurses, and CNA's in concert with a critical care physician.

    Welcome to The Center for Outpatient Therapy.....

    Located in Crossville, Tennessee the Center is an integral part of the WyndRidge campus of healthcare, long-term care, rehabilitation, and assisted living. Our healthcare network includes services beginning with Cumberland Ridge Assisted Living Center, WyndRidge Health & Rehabilitation Center, and The Center for Outpatient Therapy. We are proud to be the premier provider of a continuum of care in Middle and East Tennessee.
    The Center offers physical, occupational, and speech therapy services for individuals ranging from infants to adults. Our team of licensed therapist work with each patient, and his-or-her physician to design an individualized, outcome-oriented treatment plan.

    Physical Therapy

    Physical Therapy can foster patient's independence and enable functional mobility skills following, or in spite of, orthopedic or neurological conditions and injuries.

    Occupational Therapy

    The goal of Occupational Therapy is to help regain functional skills that may have been compromised by illness or injury.

    Speech Therapy

    The goal of Speech Therapy is to help achieve maximum communication skills in speaking, self-expression, and understanding.

    WyndRidge prides itself on the extensive activities and services it provides.

    Our residents love to dress up for holidays, costume parties, and to take trips away from the facility such as to the Crossville Air Show. Weather permitting we take a weekly bus trip around Cumberland County with stops for ice cream or sodas. We encourage those who are able to interact and visit with their new found friends and "family" here at WyndRidge.

  • A Foundation of Care:

    The Senior Living Foundation is a 501c3 charity formed to address these cost issues and to give back to a Middle Tennessee community that struggles to care for its own. An Advisory Board comprised of local clinical and business leaders has been launched to ensure a meaningful framework for much needed support.

    The Senior Living Foundation has one goal, to serve our community with the best in long term and rehabilitative healthcare for seniors and those in need, while continuing our focus on fiscal responsibility, and the proper stewardship of the monetary and in-kind donations of our supporters, donors and fellows. We can achieve this goal with the help of the community and YOUR support of The Senior Living Foundation which translates directly to care for real people with critical needs in our community. In addition to activities such as the Wounded Warrior Project, the Foundation can also provide access to clinical staff education as well as capital improvements to WyndRidge Health & Rehabilitation Center and their campus of affiliated healthcare providers.

    The Real Cost of Care:

    In 1995, nearly 40% of long-term care costs were paid for by the elderly and their families, while Medicare and Medicaid paid 60%. Today, the gap between paid health care and unpaid health care is soaring. Bankrupt seniors and the families who care for them are left without the care they need. Over 70% of single individuals and 50% of couples with one partner in a nursing home are impoverished within one year. The average monthly cost of care in a skilled nursing reaches $5,000.00. With inadequate reimbursement, skilled nursing facilities are faced with critical gaps in their ability to fund life supporting care.

  • Medicare Coverage

    What makes a patient eligible for Medicare Part A in a skilled nursing facility?

    1. The patient must have a Medicare card that reads Hospital Insurance.
    2. The patient’s physician must certify that the patient needs skilled care on a continuing basis.
    3. A minimum of three consecutive days (not counting day of discharge) must be spent in a hospital no longer than 30 days prior to entering the skilled nursing facility.
    4. The need for skilled care must relate to the reason for hospitalization.

    What is paid for under Medicare Part A while in a skilled nursing facility?

    1. Semi-private rooms
    2. Meals
    3. Nursing Services
    4. Rehabilitation Services
    5. Medication
    6. Supplies and Medical Equipment

    What are the Medicare rates if the qualifying criteria is met?

    1-20 days: Patient pays nothing, Medicare pays in full.
    21-100 days: Daily co-insurance rate is determined by Medicare.
    100+ days: Medicare no longer pays.

    Medicare Part B

    If you have medical coverage under Medicare Part B and you are eligible for services provided, the patient will be responsible for 20% of the total charges of some of the following services:
    1. Occupational Therapy
    2. Physical Therapy
    3. Speech Therapy
    4. Medical Supplies

    Since opening the unit, referrals have come from local sources, along with hospitals across the Southeastern United states. Because WyndRidge is contracted with the Veterans Administration, veterans with a service connected disability have also been admitted. Care is provided for the short-term, but also for the long-term if needed.

    This little town has a big resource that is much needed in the community as seen by the amount of referrals received in the past year. WyndRidge is proud to be in the forefront of care and looks forward to continued success as a skilled and long-term care nursing facility with a reputation spanning almost 50 years in the Crossville community.
